We are proud to support the NHS Wales, Test, Trace, Protect service in controlling the spread of COVID-19.

We maintain records of staff, customers and visitors to our premises for a period of 21 days.  After 21 days this information will be deleted.  By submitting this form you are accepting that entering the premises are at your own risk.

This data is being collected in the interests of protecting the NHS in Wales and saving lives.

It will only be used to help trace people who may have been exposed to the virus, should an outbreak occur on our premises.
